A case of ranula-homoeopathic approach

ABSTRACT:
A surgical case of ranula presented with painless reddish blue translucent  swelling below the tongue on the floor of mouth since one month was cured in 2 weeks with homoeopathic medicine selected based on symptom similarity.

PRESENTING COMPLAINT:
A 7 year old girl complained with painless reddish blue translucent swelling beneath the tongue on the floor of mouth since one month. The swelling developed gradually and has increased in size in last two weeks.

PAST HISTORY: No history of past illness

FAMILY HISTORY: Grandfather (paternal): Hypertensive

PHYSICAL GENERALS:

Appetite: Decreased (since 2 weeks)           Bowel: Regular (one time/ day)

Thirst: Increased (since 2 weeks)                              Bladder: 6 to 7 times/ day

Sleep: Refreshed (8 hours / day)           Sweat: Generalised

GENERAL PHYSICAL EXAMINATION:

There are no signs of pallor, icterus, cyanosis, clubbing, oedema or lymphadenopathy

Weight: 18 kg Height: 119 cm 

Pulse rate: 74/ min          Respiratory rate: 18/ min        Temperature: Afebrile

LOCAL EXAMINATION

Examination of mouth: Swelling of about 2 cm which translucent and reddish blue in color is present beneath the tongue (left side) on the floor of mouth.

REPERTORIAL TOTALITY: 

Repertory chosen: KENT (based on characteristic particulars)

  1. MOUTH-RANULA
  2. MOUTH-RANULA-bluish-red
  3. STOMACH-APPETITE-diminished
  4. STOMACH-THIRST
  5. GENERALITIES-SIDE- left

FIRST PRESCRIPTION:               14.11.2018

   1. Thuja 200 / 4doses (EMES)

   2. Rubrum 30/ TDS for 4 days 

FOLLOW UP 1               18.11.2018

Swelling reduced in size

Bluish red colour reduced

Thirst increased

Appetite decreased

Rx

    1. Thuja 200/ 3 doses/ alt days (EMES)

    2. Rubrum 30/ TDS for 1 week

FOLLOW UP 2                           26.11.2018

Swelling reduced to less than 1 cm

Bluish red colour changed to pale translucent colour

Appetite improved, Thirst increased

     Rx

     1. SL-2 doses/ alt days (EMES)

     2. Rubrum 30/ TDS for 1 week

FOLLOW UP 3                 3.12.2018

Swelling was not present

Normal colour of floor of mouth

Appetite improved, Thirst adequate.

CONCLUSION:

In the above case of ranula, Thuja occidentalis selected on the basis of acute totality was found to be effective in bringing about the cure in 2 weeks of time.
