Homoeopathic Management of Premature Ejaculation

Abstract
Premature ejaculation (PE) is one of the most common male sexual dysfunctions, affecting physical and psychological well-being. This case report presents a 35-year-old male with a history of PE, treated successfully with Lycopodium 200. A six-month follow-up demonstrated significant improvement in ejaculatory control and overall mental well-being, highlighting the role of individualized homoeopathic treatment in addressing psychosexual disorders.

Introduction
Premature ejaculation (PE) is defined as ejaculation that occurs with minimal stimulation before or shortly after penetration, leading to distress and interpersonal difficulties. It can be primary (lifelong) or secondary (acquired), and is influenced by psychological, neurological, and hormonal factors. Conventional treatment options include selective serotonin reuptake inhibitors (SSRIs), behavioral therapy, and topical anaesthetics, but these often provide only temporary relief. Homoeopathy offers a holistic approach by treating the underlying causes, including psychological factors such as performance anxiety, low self-esteem, and suppressed emotions.

History of Present Illness
The patient presented with persistent premature ejaculation for the past 5 years, leading to dissatisfaction in his marital life. He reported:

  • Ejaculation within 1-2 minutes of penetration.
  • Increased anxiety and self-doubt regarding sexual performance.
  • Lack of confidence and avoidance of sexual activity.
  • Episodes of erectile weakness after repeated failures.

Treatment History

  • Had previously used SSRIs and topical creams, which provided temporary relief.
  • Underwent counseling and behavioral therapy, but improvement was minimal.
  • No significant history of chronic illness or major surgeries.

Family History

  • No family history of sexual dysfunction.
  • Father had a history of hypertension and diabetes.

Physical Generals

  • Appetite: Normal, prefers warm food.
  • Thirst: Drinks large quantities of water.
  • Stool: Constipated, requires straining.
  • Urine: Normal frequency.
  • Sleep: Disturbed due to anxiety.
  • Thermal Reaction: Prefers warm surroundings, dislikes cold.

Mental Generals

  • Anxious personality, lacks self-confidence.
  • Fear of failure, avoids social interactions.
  • Ambitious yet doubtful about his abilities.
  • Easily irritated, gets angry when contradicted.
  • Strong craving for sweets.

Totality of Symptoms

  • Performance anxiety and anticipatory fear before intercourse.
  • Weakness of genital organs, inability to maintain erection.
  • Aggravation in the evening, especially after mental exertion.
  • Low self-confidence despite intellectual capabilities.
  • Constipation with flatulence and bloating.

Medicine Prescribed

  • Lycopodium Clavatum 200 (Single dose)
  • Saccharum Lactis (Placebo) once daily for follow-up

Follow-Up for 6 Months

Month  Observations

1st Month         Initial aggravation, slight improvement in confidence

2nd Month        Anxiety reduced, slight improvement in ejaculatory control

3rd Month        Ejaculation improved to 3-4 minutes, better erection quality

4th Month         Sexual confidence restored, relationship satisfaction increased

5th Month         Ejaculation within normal range (5-7 minutes), no anxiety before intercourse

6th Month         No recurrence of symptoms, overall well-being improved

Conclusion
This case illustrates the efficacy of homoeopathy in treating premature ejaculation by addressing both the psychological and physiological aspects of the condition. Lycopodium 200 was selected based on the totality of symptoms, particularly the lack of confidence, anticipatory anxiety, and digestive disturbances. Over six months, the patient experienced significant improvement in ejaculatory control, self-confidence, and overall quality of life. Homoeopathy provides a holistic and individualized approach to psychosexual disorders, offering long-term relief without side effects.
