Understanding the symptoms of first grade remedies of Mania from Kent’s repertory

ABSTRACT
Repertory  is a systematically arranged index to the Homoeopathic Materia medica which is full of information collected from drug proving, toxicological source and clinical experience.

It is a tool in which we find symptoms in the form of rubrics along with a group of medicine which are useful for prescription.

Mania is a state of elevated energy , mood and behaviour. It is a symptom seen in various psychiatric condition like bipolar disorder and schizoaffective disorder. Mania can affect one’s ability to work , to do day to day activities over a certain period of time.

This condition can be managed by Homoeopathic medicines. There are various repertories which mention about the rubric mania one such repertory is Kent’s repertory of the Homoeopathic Materia Medica.

In Kent’s repertory we find direct rubric mania in the chapter mind. It has first , second and third grade remedies .We need to differentiate remedies given under rubric  mania with Materia medica knowledge to arrive at a right prescription.

In this article an attempt has been made to differentiate the first grade remedies of the rubric Mania from Kent’s repertory.

INTRODUCTION
Mania is a state of elevated energy, mood & behaviour. It is a symptom, seen in patient with Bipolar disorder, Schizo affective disorder and also in patients who are under certain medications.

The Etymology of Mania-Ancient Greck Mania (mania) – Madness, frenzy

Mania- It is a psychological condition that causes a person to experience unreasonable euphoria , very intense moods , hyperactivity and delusion,

CAUSES

It can be caused by combination of Genetic, Chemical and Environmental factors.

  • Family History of mania or mental illness like Bipolar disorder.
  • Hormonal / chemical imbalance is the brain

(adrenaline, serotonin, dopamine)

  • Mental health conditions

Bipolar, schizo affective, Postpartum psychosis

  • Neurological Conditions

Brain injury, tumour, stroke, encephalitis, dementia.

  • Insomnia
  • Alcohol abuse
  • Recreational drug abuse
  • Side effects a medications
  • Traumatic life event

Mania can be presented in 3 different Stages:-

1. Hypomania-Stage 1

it is a milder form than other two stages.

It may not be recognized as a symptom by those around the person experiencing it. It affects sleep and activity and may cause increased impulsivity.

2.Acute mania-Stage 2

Patient acts in an inappropriate and impulsive manner

Increased energy , little to no sleep , talks hastily and loquacious jumping from one subject to another.

Not fully aware of reality

3.Delirious mania-Stage 3

Most severe of the three stages.

Symptoms are similar to mania but with addition of delirium.

Combination of mania and psychosis

Patients need hospitalization

The hallmark of mania incudes rapid speech, increased talkativeness, crowding of thoughts , unable to focus, reduced sleep and psychomotor agitation .

The rubric Mania contains 10 first grade remedies

They are as follows-

  • Arsenic album
  • Belladona
  • Mercurius
  • Hyoscyamus
  • Nux Vomica
  • Kali Bromatum
  • Kali Chloricum
  • Stramonium
  • Veratrum album

1) ARSENIC ALBUM

  • The mania of arsenic album is intermingled with fear , anxiety , impulses , suicidal inclinations , sudden freaks .
  • The states of anxiety & restlessness go towards the cadaveric aspect, towards death.

2) BELLADONA

  • Acute mania – patient will bite the spoon, will bark like a dog, will do all sorts of violent thing, even jump out of the window.
  • All sorts of delusions & hallucinations are mingled with acute mania, ghosts, horrid monsters, strange things and deformed subjects.
  • Violence, Rage, excitement well be seen in mania.

3) HYOSCYAMUS

  • Passive mania, does not go into violence.
  • More likely to be suicidal than committing a murder.
  • Sometimes the patient will talk & prattle, sometimes sit & say nothing.

4) KALI BROMATUM

  • Suicidal mania with trembling

5) KALI CHLORICUM

  • Liveliness then ill humour
  • Ill humoured , anxious.

6) LYCOPODIUM CLAVATUM

  • Excessively merry and laugh at simplest things.
  • A condition of insanity.

7) MERCURIUS

  • Hastiness ;a hurried restless anxious disposition .
  • Desire to kill people contradicting her. Impulse to commit suicide , sudden anger with impulse to do violence .
  • Fearful that she will lose her reason and carry the impulse out.

8)NUX VOMICA

  • Oversensitiveness ,touchy, irritable.
  • They are never contended, never satisfied disturbed by their surroundings and they become irritable , so that they want to tear things, to scold.

9)  STRAMONIUM

  • It is useful in mania that has existed for sometime ;attacks of mania coming on in paroxysms, appearing with more or less suddenness .
  • The mind is in uproar ;cursing , tearing the clothes , violent speech , frenzy , erotomania , exposing of the person.

10)  VERATRUM ALBUM

  • Mania with desire to cut and tear everything, especially the clothes, with lewdness and lascivious talks .
  • Puerperal mania and convulsions with violent cerebral congestion.
  • She is inconsolable over a fancied misfortune, runs around the room howling and screaming or sits brooding, wailing and weeping.

CONCLUSION
Mind chapter of Kent’s repertory is full of rubrics useful for various psychiatric symptoms and condition. One such symptom is Mania.In this article an attempt has been made to discuss in brief about mania and then differentiate the first grade remedies in rubric mania of Kent’s repertory.
