class=\"p3\"><span class=\"s2\"><b>INTRODUCTION<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li7\"><span class=\"s1\"><b>Cardiomyopathy<\/b>\u00a0is a disease of the heart muscle that makes the heart harder to pump blood to the rest of our body.\u00a0<b>Cardiomyopathy<\/b>\u00a0can lead to heart failure.<\/span><\/li>\n<\/ul>\n<p class=\"p8\"><span class=\"s2\">There are 5 types of cardiomyopathy among which first three are commonly seen<\/span><\/p>\n<ol class=\"ol1\">\n<li class=\"li8\"><span class=\"s2\">Dilated cardiomyopathy<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Hypertrophic cardiomyopathy<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Restrictive cardiomyopathy<\/span><\/li>\n<\/ol>\n<p class=\"p9\"><span class=\"s2\">Rare types are<\/span><\/p>\n<p class=\"p9\"><span class=\"s2\">4.<span class=\"Apple-converted-space\">\u00a0 <\/span>Arrhythmogenic right ventricular cardiomyopathy<\/span><\/p>\n<p class=\"p9\"><span class=\"s2\">5.<i> <\/i>Obliterative cardiomyopathy<\/span><\/p>\n<p class=\"p10\"><span class=\"s2\"><b>DILATED CARDIOMYOPATHY<\/b><\/span><\/p>\n<ol class=\"ol1\">\n<li class=\"li8\"><span class=\"s2\">This is characterised by <b><i>dilatation and impaired contraction <\/i><\/b>of Left Ventricle (and sometimes the Right Ventricle). <\/span><\/li>\n<li class=\"li8\"><span class=\"s2\"><b><i>Left ventricular area is increased <\/i><\/b>but thinning is normal or reduced.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">This condition affects mostly adults.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\"> The average survival rate from the onset to death is less than 5years.<\/span><\/li>\n<\/ol>\n<p class=\"p11\"><span class=\"s2\"><b>Etiology<\/b><\/span><\/p>\n<ul class=\"ul2\">\n<li class=\"li8\"><span class=\"s2\">High blood pressure<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Previous heart attacks<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Alcohol\/Cocaine use<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Toxins (lead, mercury)<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Infections<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Obesity<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Diabetes<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Pathogenesis<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Thinning of ventricular walls along with dilatation.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Endocardium becomes tinner and mural thrombi are often found in ventricles and atria (cardiac valves are normal)<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Degenerative changes with small areas of interstitial fibrosis are found with focal mononuclear inflammation of cell infiltrates. <\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Histological changes include <b>myofibrillary loss, <\/b>interstitial fibrosis and T-cell infiltrates.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Heart is enlarged and increase in weight of heart seen upto 100gms.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Prominent dilatation of all 4 chambers giving globular appearance to the heart. <\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Clinical features<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Frequent chest pains<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Arrhythmias <\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Thromboemobolism <\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Sudden death<\/span><\/li>\n<\/ul>\n<p class=\"p13\"><span class=\"s2\"><b>Investigations<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">ECG usually shows non-specific changes but <b><i>Echocardiography is useful in establishing diagnosis. <\/i><\/b><\/span><\/li>\n<\/ul>\n<p class=\"p13\"><span class=\"s2\"><b>Management<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Risk can be reduced by rigorous medical therapy with \u03b2-blockers and angiotensin receptors antagonists.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Some patients may be considered for implantation of cardiac defibrillator and\/ or cardiac resynchronisation therapy.<\/span><\/li>\n<\/ul>\n<p class=\"p10\"><span class=\"s2\"><b>HYPERTROPHIC CARDIOMYOPATHY<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">This is the <b><i>most common <\/i><\/b>form of cardiomyopathy, characterised by <b><i>inappropriate and elaborate left ventricular hypertrophy.<\/i><\/b><\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Its also known as Assymetrical hypertrophy, Hypertropic Subaortic stenosis and Tear\u2019s disease<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">The hypertrophy may be generalised or confined largely to the interventricular septum (asymmetric septal hypertrophy) other regions (apical hypertrophy cardiomyopathy)<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Pathogenesis<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">It\u2019s a genetic disorder usually with autosomal dominant transmission<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\"> a high degree of penetrance and variable expression.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Marked myocardial fibre disarray<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Abnormal vascular response (eg. Exercise induced hypotension)and high risk of sudden death.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Hypertrophy of myocardium is assymetrical and affects interventricular septum.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Its seen in apical region of the septum extended up to the mitral valve <b><i>causing obstruction to left ventricular outflow <\/i><\/b>in the form of subaortic stenosis. <\/span><\/li>\n<\/ul>\n<p class=\"p14\"><span class=\"s2\"><b>Clinical features<\/b><\/span><\/p>\n<p class=\"p15\"><span class=\"s2\"><b>Symptoms<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Angina on effort<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Dyspnoea on effort<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Syncope on effort<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Sudden death<\/span><\/li>\n<\/ul>\n<p class=\"p15\"><span class=\"s2\"><b>Signs<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Jerky pulse<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Palpable left ventricular hypertrophy<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Double impulse at the apex (palpable 4<\/span><span class=\"s7\"><sup>th<\/sup><\/span><span class=\"s2\"> heart sound due to left atrial hypertrophy)<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Midsystolic murmur at the base<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Pansystolic murmur (due to mitral regurgitation ) at the apex.<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Investigations<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">ECG- abnormal and shows features of left ventricular hypertrophy with wide variety of often bizarre (unusual) abnormalities (eg: pseudo infarct pattern, deep T-wave inversion).<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Echocardiography is a diagnostic, although the diagnosis may be difficult when another cause of left ventricular hypertrophy is present (eg: physical training- athlete&#8217;s heart, hypertension) but the degree of hypertrophy is greater than expected. <\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Genetic testing may facilitate diagnosis.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">The natural history is variable but clinical deterioration is often slow.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">The annual mortality from sudden death is 2-3% among adults and 4-6% in children and adolescents.<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\"><b>Sudden death <\/b>occurs typically during or just <b>after vigorous physical activity;\u00a0<\/b><\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Hypertrophic cardiomyopathy is the most common cause of sudden death in young athletes.<\/span><\/li>\n<li class=\"li16\"><span class=\"s2\">Ventricular arrhythmias are thought to be responsible for many of the deaths.<\/span><\/li>\n<\/ul>\n<p class=\"p17\"><span class=\"s2\"><b>RESTRICTIVE CARDIOMYOPATHY<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">This is a rare condition<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Here ventricular filling is impaired because the ventricles are <b>\u2018stiff\u2019<\/b><\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">This leads to high atrial pressure with atrial hypertrophy, dilatation and later atrial fibrillation.<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Eitiology <\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Amylodosis is the most common cause of restrictive cardiomyopathy in UK.<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>Other causes<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Glycogen storage disease<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Hereditary causes<\/span><\/li>\n<\/ul>\n<p class=\"p11\"><span class=\"s2\"><b>INVESTIGATIONS REQUIRED<\/b><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Diagnosis may be very difficult and requires complex Doppler echocardiography, CT, MRI and endomyocardial biopsy.<\/span><\/li>\n<\/ul>\n<p class=\"p10\"><span class=\"s2\"><b>HOMOEOPATHIC APPROACH<\/b><\/span><\/p>\n<p class=\"p13\"><span class=\"s2\">According to Hahnemannean classification of diseases<\/span><\/p>\n<p class=\"p13\"><span class=\"s2\">The types of cardiomyopathy that falls under True Chronic diseases are <\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Primary cardiomyopathy<\/span><\/li>\n<\/ul>\n<ol class=\"ol1\">\n<li class=\"li8\"><span class=\"s2\">Idiopathic dilated cardiomyopathy<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Idiopathic hypertrophic cardiomyopathy<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Idiopathic obliterative or restrictive cardiomyopathy<\/span><\/li>\n<\/ol>\n<p class=\"p13\"><span class=\"s2\">Secondary cardiomyopathy falls under pseudochronic disease according to Hahnemannean classification of diseases<\/span><\/p>\n<p class=\"p13\"><span class=\"s2\">Following are the causes for <\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li8\"><span class=\"s2\">Secondary<span class=\"Apple-converted-space\">\u00a0 <\/span>cardiomyopathy <\/span><\/li>\n<\/ul>\n<ol class=\"ol1\">\n<li class=\"li8\"><span class=\"s2\">Nutritional disorders<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Toxic chemicals<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Drugs<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Metabolic diseases<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Neuromuscular diseases<\/span><\/li>\n<li class=\"li8\"><span class=\"s2\">Connective tissue diseases<\/span><\/li>\n<\/ol>\n<p class=\"p10\"><span class=\"s2\"><b>HOMOEOPATHIC APPROACH FOR CARDIOMYOPATHY<br \/>\n<\/b><\/span><span class=\"s2\">Homoeopathy is a system of medicine which is based on \u2018Law of Similars\u2019 and the patients are being treated through holistic approach by individualizing their characteristic symptoms and through symptom similarity wherein chronic diseases of this type can be better approached constitutionally by considering the totality of symptoms and for some of the
